The American College of Emergency Physicians (ACEP) gratefully acknowledges the many organizations and government agencies listed in the appendix that had collected and published data that were instrumental in the creation of this report.

ACEP particularly appreciates the valuable contribution of the Safe States Alliance for permitting the use of the results of injury prevention funding questions from its 2011 “State of the States” survey of injury prevention directors and the American Hospital Association (AHA) and Health Forum, LLC for use of aggregate 2011 AHA Annual Survey data related to health care facilities.
